rates are guaranteed until December 31, 2011
- Enrolled
members and law firms must commit to a 24 months of coverage (a
standard requirement for group vision plans).
- Sole
proprietor may enroll if the completed forms is received in
our office within 30 days of membership. Coverage
will become effective on the 1st of the month following date of
membership. If completed applications with payment
are not received within that time, January 1, of the coming year
will be the next open enrollment opportunity. If a sole
proprietor hires employees, the employees must be enrolled in
the plan.
- 100% of
eligible (full-time working 30+hours) employees and dependents
must enroll except for those who have vision coverage through a
spouse plan. Employees must be enrolled within 30 days of
their hire date.
- Employers
pay 100% of their employees' premium.
- Enrolled
members will receive a plan booklet and wallet-size laminated
card with VSP phone numbers and Web site. Check the VSP
website www.vsp.com for the
participating providers in your area. When
non-participating providers are used, lesser benefits will
apply.
